Durham Community Health Network (a Carolina Access II Program) Eligibility Criteria:

Patient is enrolled in Medicaid through the Carolina Access program and would benefit from having a care manager help with any of the following:

Missed medical appointments or frequent Emergency Room use

Family problems that affect his/her ability to care for health concerns

Coordination of multiple healthcare services and needs

Education and resources for a chronic health condition, such as asthma or diabetes

 

4 County Community Care Eligibility Criteria:

Patient is enrolled in Medicaid through the Carolina Access program, is a resident of Person, Warren, Vance or Granville county, and would benefit from having a care manager help with any of the following:

Missed medical appointments or frequent Emergency Room use

Family problems that affect his/her ability to care for health concerns

Coordination of multiple healthcare services and needs

Education and resources for a chronic health condition, such as asthma or diabetes

 

Central Carolina Health Net Eligibility Criteria:

Patient is enrolled in Medicaid through the Carolina Access program, uses a clinic in Alamance, Caswell, Chatham or Orange counties, and could use care management help with:

Missed medical appointments or frequent Emergency Room use

Family problems that affect his/her ability to care for health concerns

Coordination of multiple healthcare services and needs

Education and resources for a chronic health condition, such as asthma or diabetes

 

BabyLove Eligibility Criteria:

Patient is pregnant and:

Receives or may be eligible for Medicaid

 

HealthCheck Eligibility Criteria:

Child is less than 21 years of age, receives Medicaid and (either of the following):

Is missing well-care appointments

Parent/guardian of the child needs further education on the appropriate use of primary care

 

 

Child Services Coordination (CSC) Eligibility Criteria:

Child is between 0-5 years of age (Medicaid or non-Medicaid) and:

Would benefit from a nurse of social worker to make health or developmental assessment and to assist with coordination of services, education, and parenting skills.

 

Community Alternatives Program for Medically Fragile Children (CAP/C) Eligibility Criteria:

Child is under 19 years old and lives in a private residence and:

Has medical needs which require intermediate or skilled level of care, AND

Requires Medicaid coverage of services & case management to remain safely at home

 

LATCH (Local Access to Coordinated Health Care) Eligibility Criteria:

Patient lives in Durham County and:

Does not have private insurance or is under-insured, AND

Does not receive Medicare or Medicaid benefits
